Project Overview
Zeihan ProHealth partnered with Blue Cross Blue Shield of Arizona Health Choice to implement a comprehensive outreach and preventive screening initiative to expand access to care for Medicaid members across Arizona. The initiative delivered culturally responsive health education, biometric screenings, and care navigation services across urban and rural communities.
Through collaboration with community-based organizations and healthcare partners, the program strengthened linkages to Federally Qualified Health Centers (FQHCs), increased awareness of Medicaid resources, and supported enrollment into the Arizona Health Care Cost Containment System (AHCCCS). These efforts advanced equitable access to preventive care, supported improvement in quality measures and preventive care metrics, and reinforced a statewide commitment to community-centered health.
